Step 2: Sign and Return Before Your Lesson

To complete your registration, please follow these simple steps before your first private lesson:

Sign by Hand: Fill out the Signature, Printed Name, and Date fields at the bottom of the printed page

Take a Photo or Scan: Use your smartphone to take a clear, well-lit photo of the entire signed page, or scan it

Email It Back: Send the photo or scan as an attachment directly to sandy@acedogsports.com .

Important Note: Each client is required to have this release form on file prior to active training sessions. You only need to do this once for your very first booking, it will cover all future lessons!
